Mineral Mastery and Wellness Institute Practitioner AgreementPlease fill out the agreement form below. REGISTRANT INFORMATION* Name * First Name Last Name AGREEMENT I understand I have 1 year to complete the training. A passing score of 90% is required to obtain my certificate. If I do not pass on the first attempt, I may retake the test(s). * The course fee includes one year of personalized coaching to review HTMA results and guide me in mineral balancing, at no additional cost, as long as I order hair tests through ARL (Analytical Research Labs) and purchase supplements directly from Endo-met Labs. * If Endo-met supplements are not available in my country, one-on-one coaching is available at $60 per 30-minute session. * One-on-one chart reviews will be conducted by phone or Zoom. Email is for basic questions only, as email-based teaching is not effective for understanding mineral balancing science. * Once I log into the training platform (Kajabi), no refunds will be issued. * I will be asked to create supplement programs and interpret HTMA tests when my coach determines I am ready. The goal is to empower me as a practitioner. * One-on-one instruction begins 10 days after my certificate is issued and continues for one year. After one year, additional coaching is available at $60 per 30 minutes. * To be listed on the Global Nutritional Healing website, I must: 1. Use ARL for all hair tests 2. Use Endo-met for supplement programs (unless Endo-met does not ship to my country). * I agree all course materials—including assignments, booklets, newsletters, notes, exams, and other resources—are the intellectual property of Global Nutritional Healing (GNH). I will not share, copy, or distribute these materials. * I understand that Global Nutritional Healing is not a medical organization. We do not diagnose, treat, or cure medical conditions. The program provides educational guidance only, and I am responsible for operating legally within my scope of practice. * Mineral balancing is intended to reduce stress and balance body chemistry, not as a diagnosis, treatment, or prescription for any disease. * This program is 100% online. All course materials are included except for the book The Good Gut by Dr. Justin Sonnenburg and Erica Sonnenburg. * Upon successful completion of the online training and passing the assessments, I will receive a Certificate of Completion, which qualifies me to open accounts with ARL and Endo-met Labs.. * Signature * Have Questions?
